By signing this agreement, I and my date (if applicable), acknowledge the behavior agreement on the back of this form. If your guest does not attend Foothill High School, a Foothill administrator must sign below after you have completed the information for you and your guest. STUDENT: I realize that the dance is an important and exciting event, and at the same time, I agree that one’s own safety is vitally important. Therefore, I agree not to use drugs and alcohol prior to, during, or after the dance. Furthermore, I understand that I am accountable for my own actions. Should this agreement be violated, the consequences for Foothill High School students will be enforced per administrator discretion in accordance with the Foothill High School Discipline Plan. I further understand that if I appear under the influence of drugs or alcohol, I will not be admitted and will be detained until a parent, guardian, or authorities arrive to take responsibility. The law (SB 1300) regarding persons aged 13-20 and the possession or consumption of alcohol or drugs, will also be applied to the violation. The possible loss of your driver’s license or deterrent to receiving a license could result.
