This is the application packet and criteria for each scholarship application entry. Fill-out the entire application packet. Write neatly. Write the scholarship’s name on each application entry packet you are handing in. One complete application packet is required for each scholarship entry. Attach a copy of your current high school transcript to each application entry packet (www.parchment.com – request the “unofficial self-view transcript” - and print at home). If you can’t print one from home, let the secetary know as you hand in your scholarship packet(s). Attached required essays (include specific scholarship name on each essay.) Turn in an extra copy of the application entry packet (with transcript and all essays you might have written) for NHS to keep on file for reference. Request two NHS staff members to complete the attached recommendation forms. These forms are returned to the counseling secretary directly and not given to student. Do not ask an NHS staff member for a recommendation after February 20th! If outside letters of recommendation are requested, they can be mailed directly to Mrs. Purcell in the Counseling Office (45700 Six Mile, Northville, MI 48168) or attached to your application entry. Paper clip and DO NOT staple application packet(s). NOTE REVIEW the requirements for each scholarship entry, make sure you have all documents properly labeled and paperclipped. If you do not meet all of the qualifications, or if your application entry packet is incomplete, it will be disqualified. Student must be of sound moral/ethical character as detailed in the Student-Athlete Code of Conduct. 3. One of the two required forms MUST BE completed by a Coach/Assistant Coach (i.e.: Freshman, JV, Varsity, Travel, Club, etc.) 4. Student must write an essay detailing how athletics has made her/him the person she/he is today. Essay should be TYPED and a maximum of 1,500 words.

Must be a Northville High School student. PARENT AND STUDENT MUST be a member of the PTSA as of January 31, 2016. Minimum GPA of 2.50. Financial need may be considered. Both required recommendations must be from school personnel. Or, if attend LCTC, one recommendation can be from school personnel and one can be a LCTC teacher. Must write one essay (250 word minimum, please type) on “What about your educational experience at NHS gives you a deep sense of thankfulness and gratitude”? Write a “thank you letter” to NHS about something for which you are grateful. Feel free to include specific examples of your personal or educational experiences with a teacher, a staff member, or with your peers – and the life lessons learned through them (even the hard ones). Must be enrolled full time in a college or technical institute in the summer or fall following graduation.
